Under-served Youth Resources Montgomery County – Funded by DHHS/SAMHSA

The Montgomery County Collaboration Council requested a Geo-mapping Tool in order to assess resources for transitional age youth (TAY) who are at-risk of disconnecting or are disconnected from their communities.

This effort was funded through SAMSHA for the Montgomery County Bridges to the Future System of Care. Specifically, those youth between the ages of 16 and 21 who are unemployed, not in school or a combination of the two statuses with behavioral health challenges. Optimal was selected for the task and designed the tool for the Collaboration Council to easily access the service providers in Montgomery County, to provide referrals for services to connect youth and young adults with behavioral health challenges, and to help visualize, as well as identify gaps in services within the county.
